Receiving the hospitalization order and consent form by admission staff from patient/escort during visitation to admission unit*

Is patient able to state their full personal information?

• No: Receiving necessary information from patient’s escort
• Yes: Showing an ID card to the admission staff in order to confirm the information delivered by patient/escort.

Entering all patient’s identification information in HIS, creation of patient file, receiving necessary consent forms, providing patient with ID bracelet, escort card and hygiene item bag, and informing the escort regarding patient’s file number registered in the escort card by admission staff;

Sending the patient to the financial unit for payment and insurance processing and guiding the patient to the relevant clinical ward for initial diagnosis

After entering the relevant clinical ward, the patient will be guided by the nurse for receiving electro and change of cloths

After returning to the ward (after receiving electro ad change of clothes), clinical services will be delivered according to patient’s needs and condition

* If patient and escort are unable to visit the admission unit, admission staff will visit the patient in order to complete the admission process.
